Failure Modes and Effects Analysis (FMEA) is a systematic technique of identifying, analyzing and preventing product and process problems before they occur. When implemented properly, FMEA is one of the most valuable reliability and design tools. Improperly executed causes FMEA's to be at best a waste of resources and time and at worst, steers you in the wrong direction resulting in missed failure modes.
In this paper we perform FMEA on FMEA, exploring how FMEA's can go wrong and how to avoid these common pitfalls.
